Best Mattress for Side Sleepers With Shoulder Pain
Side sleeping is where shoulder pressure gets serious, because your whole upper-body weight lands on one joint and stays there for hours. Two things have to happen at once:
- The shoulder has to have somewhere to go. Side sleepers need a comfort layer deep enough to accept the widest parts of the body — shoulder and hip. Thin comfort layers over firm coils mean you bottom out and the joint carries the load.
- The comfort layer has to be deep enough. From fitting side sleepers in person, roughly three inches of comfort layer is the practical minimum for an average shoulder width. Thinner than that and you reach the firm support coils before the joint has finished sinking.
- The waist has to stay supported. If the shoulder sinks but the support core also gives way, the spine curves and you trade shoulder pressure for lower-back ache.
The most common mistake we see walk through the door is shoppers asking for “firm” because they equate firm with supportive. For a side sleeper with a sore shoulder, firm usually equals pressure.
That combination is why zoned or micro-coil hybrids and latex hybrids dominate our list: they immerse locally and support globally. It’s also why very firm beds so often make shoulder discomfort worse, even though “firm equals supportive” is the most common assumption we hear in the showroom.
Two practical notes from fitting side sleepers in person at our Encinitas showroom:
- The bed that feels supportive under a standing hand often feels punishing after twenty minutes on your side. Lie down in your actual sleeping position, with your own pillow if you can, for at least ten minutes per mattress.
- Responsiveness matters as much as softness for a sore shoulder. If you need to switch sides to unload the joint, a surface that holds you in place makes that harder.

The shoulder-relief fit number is a weighted blend of two scores we already publish for every mattress: pressure relief (60%), which is how the shoulder and hip settle into the comfort layers on a hands-on side and back test, and spine alignment (40%), which is whether the spine stays neutral once they do. Beds firmer than medium-firm are excluded, because a stiff surface is what pins a sore shoulder in the first place. It is a ranking aid built from those published scores, not a separate instrument reading taken at the shoulder.

Firmness by body weight
Body weight changes how far you press into any surface, so the same mattress is a different bed for two people. Using Yawnder’s standard firmness scale (1 = softest, 10 = firmest):

These are starting points, not rules. Shoulder width, hip-to-waist ratio and whether you sleep with an arm tucked under the pillow all shift the answer, which is why a long sleep trial matters more than any chart.
Pillow loft matters too (but the mattress comes first)
Mattress feel and pillow height work as one system for side sleepers. A well-chosen mattress lets the shoulder drop; the pillow then has to fill the remaining gap between ear and surface so the neck isn’t craned up or folded down. If your pillow is too flat, your head tips toward the mattress and the shoulder rolls forward under you; too tall, and the neck angles the other way. Broader shoulders generally need more loft, and a plusher mattress needs slightly less because the shoulder is already sinking.

What firmness is best for shoulder pain?
Medium (5–6.5) for most average-weight side sleepers, plush (3–5) under about 130 lb, and medium-firm with a reinforced support core above about 230 lb. The aim is enough local give for the shoulder with the waist still supported.
Is memory foam or a hybrid better for shoulder pressure?
Both can work. Memory foam gives the deepest, slowest cradle but can feel warm and make turning over harder. Hybrids and latex hybrids relieve pressure and rebound, which suits anyone who changes sides to take weight off a sore shoulder.
How firm is too firm?
If you wake with a numb arm or a sharp point of pressure at the shoulder, the surface probably isn’t letting the joint immerse. That’s a comfort-layer depth problem, not a support problem, and going firmer usually makes it worse.
How long should I give a new mattress?
Most people need about three to four weeks to adapt to a new surface, which is why we weight long sleep trials heavily. The trials in the table above run from 100 to 365 nights.
Will a mattress topper fix a too-firm bed?
A 3-inch memory foam or latex topper can add shoulder immersion to a bed that’s too firm, but it can’t fix a sagging support core underneath. If your mattress is more than seven or eight years old and visibly dipped, replacing beats topping.
Should I switch to sleeping on my back?
Back sleeping removes direct shoulder loading, so it helps some people. Forced position changes rarely stick overnight, though, so most side sleepers do better choosing a surface that suits side sleeping.
Can a mattress treat a rotator cuff injury or bursitis?
No. A mattress can reduce pressure on the shoulder and help you stay in a neutral position; diagnosing and treating a shoulder injury is a clinician’s job. If pain wakes you regularly or is getting worse, get it looked at.
