What actually causes nighttime hip pain

Three things, almost always in this order:

  1. Direct pressure on the trochanter. If you side-sleep on a firm mattress, your bodyweight rests on a 4-inch bony point. After two hours the bursa inflames and you wake to roll over.
  2. Lumbar collapse. If the mattress sags under the hip but doesn’t support the lower back, the pelvis tilts and the SI joint takes the load. This shows up as “hip pain” but it’s actually back pain referred forward.
  3. Bottoming out. Heavier sleepers compress past the mattress’s useful range. The hip ends up resting on the springs or the foundation. This is why a $1,200 mattress that works at 160 lb fails completely at 240 lb.

Is a soft or firm mattress better for hip pain?
