Why the filter is stricter than the back-sleeper ranking

A back sleeper’s weight is spread along the whole spine; a stomach sleeper concentrates it at the pelvis with nothing above to counterbalance. That is why the firmness floor here is medium-firm rather than medium, why spine alignment carries more weight in the blend, and why all-foam builds are excluded outright — a foam core that bottoms out under the hips is exactly the failure this page exists to avoid. How firm should a mattress be for a heavy stomach sleeper?

Should I use a pillow under my hips?

A thin one can help on a bed that is slightly too soft, by lifting the pelvis back level with the chest. It is a patch, though — if you need it every night, the mattress is the problem, and the picks above are chosen to make it unnecessary.

Is a firm mattress bad for pressure points when I sleep on my stomach?

Less than you would think. Stomach sleeping spreads weight across the chest, hips and thighs rather than concentrating it on a shoulder, so pressure relief matters far less than in the side-sleeper rankings. Support and alignment are what keep you comfortable here.
