Why your shoulder hurts at night

Shoulder pain in bed usually comes from one of three mechanical problems, and each has a different fix. If the pain is in the shoulder you sleep on, the joint is being compressed — the issue is usually mattress support and pillow height, not the pillow's material. If the pain is across the top of the shoulder and into the neck, the head is likely tipping too far to one side because the pillow is too flat or too tall. If it wakes you when you roll, the surface is probably too firm to let the shoulder sink in at all.

Getting pillow height right

The goal is a neutral neck: ears roughly in line with the shoulders, nose pointing straight ahead, no crease at the side of the neck.

  • Side sleepers need the most loft — commonly 5 to 6 inches — to fill the gap between the ear and the outside of the shoulder. Broader shoulders need more.
  • Back sleepers need medium loft, roughly 3 to 5 inches, with some support under the neck curve.
  • Stomach sleepers need very little, 2 to 3 inches or none. Stomach sleeping is the position most likely to aggravate a shoulder, and switching positions is often more effective than any pillow.

Adjustable shredded-fill pillows are the practical choice when you are not sure, because you can remove fill in small amounts over a week rather than guessing once. Solid latex holds height consistently and does not need fluffing. Memory foam contours closely but softens in a warm room, which lowers effective loft. Down and down-alternative compress the most, so buy them taller than you think you need.

The second pillow trick

Two additions help more than most people expect. Side sleepers: hug a pillow to your chest so the top arm is supported and the shoulder is not rolled forward. Back sleepers: put a thin pillow or rolled towel under the upper arm so the shoulder is not hanging back into the mattress.

The mattress is half the problem

No pillow fixes a mattress that will not let your shoulder sink. Side sleepers with shoulder pain usually do better on a medium to medium-soft surface with a genuine comfort layer — foam, latex or a thick pillowtop — over supportive coils. A too-firm bed forces the shoulder up into the joint; a too-soft bed lets the torso sag so the shoulder collapses forward. If your current bed is firm and otherwise fine, a 2 to 3 inch topper is a cheaper test than replacing it.

When to see a professional

Night pain is a documented feature of rotator cuff problems, frozen shoulder and bursitis, and bedding cannot treat any of them. See a clinician if the pain wakes you regularly, you cannot lift your arm overhead, there is weakness or numbness down the arm, or it followed an injury. Should I sleep on my back if my shoulder hurts?

If you can, yes — it takes direct load off both shoulders. Support the arms slightly so they are not falling backward, and expect it to take a few weeks to feel natural.

Is memory foam or latex better for shoulder pain?

Latex keeps its height and sleeps cooler; memory foam contours more closely and quiets pressure points. Either works if the loft is right for your shoulder width, which matters more than the material.

Do wedge or positioning systems help?

Multi-piece positioning systems can help committed side sleepers by keeping the shoulder from rolling forward, but they take up a lot of bed and take some getting used to. Try a body pillow first.

How long before a new pillow helps?

Give it two weeks. New loft changes how your neck sits, and it is common to feel slightly worse for a few nights before it settles.
